After rescuing a woman from a demon, Johnny Blaze recounts how he came to be the Ghost Rider in a bar, when a man named Adam offers to rid him of the curse now that the Serpent is spreading fear across the world.
Johnny refuses and sees people rioting outside, including a man assaulting a woman. He goes to help her without changing into the Ghost Rider and takes a beating from him. Waking up in the woman's home, he is reluctant to accept help from her, believing that no one helps him without wanting something in return.
Johnny changes his mind and asks Adam to take the curse away, the catch being that someone Johnny doesn't know will become the Ghost Rider in his stead. Now all Johnny has to do is drive so fast that the curse rips off him. He succeeds and finds himself in the desert, finally free of the Ghost Rider.

In Dayton, Ohio, Skadi, daughter of the Red Skull and current follower of the Serpent, leads an all-out assault while Blackout and Deathwatch observe from a distance, planning to make their move soon. A female Ghost Rider appears to challange them, holding the handlebar of her motorcycle, which turns into the the rest of her bike as she drives past them, an attack that makes them experience the suffering that they have caused to others. The Ghost Rider then draws her scythe from the flames and cuts them down.
But Skadi discovers her and engages her in a fight. She is run down by the Ghost Rider's motorcycle and tied up with her chain. Ghost Rider tries to pick up Skadi's hammer so that she can kill her with it, but touching it causes her to lose consciousness and change back.
Meanwhile, Johnny Blaze returns to his cabin with firewood, where he finds Mephisto, who tells him that he has damned the human race.

Mephisto takes Johnny to Hell and asks him for help. Johnny refuses and is brought to the Serpent's lair, where Mephisto tells him about Adam's plan to save mankind. The Serpent's war is what made Adam finally decide to acquire the Ghost Rider, the greatest weapon he could find. But before he can continue to explain, the Serpent becomes aware of their presence, so Mephisto takes him to a future where Adam's plan succeeds. Adam wants to rid the world of all sin, but doing so would turn humans into mindless, emotionless beings. Mephisto needs Johnny's help because he has the most experience when it comes to controlling the Ghost Rider.
Alejandra, the new Ghost Rider, returns to the temple in Nicaragua after her battle with Skadi and is immediately ordered by Adam to wipe out the sin in her fellow students. The Seeker objects to this but is held back by the gunfire of Adam's underlings. But Alejandra is also against this and tries to fight Adam, only to be grabbed by him, putting her under his control.
Johnny, having agreed to help Mephisto, rides through Nicaragua on a motorcycle, and after entering the temple, shoots Adam in the shoulder with a hellfire shotgun. The Seeker asks Johnny to take him to the Ghost Rider so that he can power her down. Mephisto's servant, the Servicer, breaks in, but, being a creature of pure sin, Ghost Rider destroys him with a swarm of locusts that come out of her mouth. Johnny offers to help her, but still under Adam's control, she hits him and puts her scythe to his neck, threatening to wipe out his sin.
Just as the Ghost Rider is about to kill Johnny with her scythe, the Seeker steps between them and threatens to decrease her power. Adam uses his powers to toss him aside and feeds his power to the Ghost Rider.
The Seeker informs Johnny that the energy building up inside of her will soon erupt like a bomb and that they need to leave. Her power consumes everyone in the temple and their sin is erased, and when Johnny and the Seeker reach a village just outside the temple, they are all caught in the explosion.
In Colombia, Avengers Hawkeye, Ms. Marvel, and Spider-Woman, fresh from their fight with the Hulk, witness the explosion. Ms. Marvel goes to check it out and deduces that it was a energy blast.
Back in Nicaragua, everyone aside from Johnny and the Seeker have become like mindless zombies now that they are without sin. The Seeker tells him that they weren't affected because the Seeker grabbed onto him and nullified the Ghost Rider's power.
Meanwhile, Adam and the Ghost Rider break into Cape Canaveral with the intention of boarding a space shuttle.

Aboard a space station, Adam and the Ghost Rider plan on focusing her power on the station's camera lens, which would hit the entire Earth. Johnny finds Mephisto in Hell and orders him to give him a bike that he can fly into space. Mephisto agrees, but in return, Johnny must make a secret deal with him. Afterwards, Johnny and the Seeker are flying after the Ghost Rider. But Johnny realizes that the Seeker is only going to use her as a weapon and decides to eject him from the bike, sending him back to Earth. Johnny arrives at the space station but must enter by firing missiles at the entrance, causing more damage to the ship than he would have preferred.

Inside, Johnny discovers two astronauts whose sin was erased by the Ghost Rider, giving him no choice but to throw them overboard in an escape pod. Eventually, he finds the Ghost Rider's power about to explode from her. Johnny tries to convince her that it's her power to control and that it's her choice what she does with it. But Adam shoots Johnny in the stomach with his hellfire shotgun and admits to being the one who created sin and feeling guilty for it. The Ghost Rider realizes that Adam's war on sin was his attempt at fixing his mistakes and finally decides to stand against him, using her powers to blow up the space station.
Back in Nicaragua, Johnny awakes beside Alejandra, who tells him he survived because she pulled the hellfire out of him. Feeling guilty for taking the sin out of the Nicaraguans, Alejandra decides to find a way to return it to them. Johnny offers to teach her how to handle the curse, claiming to have known Ghost Riders, but she refuses, not wanting him telling her what to do.

In the deserts of Mexico, a little girl is fleeing from two men with guns. When she trips, she throws a rock at one of them in defense, an act that makes him mad enough that he decides to put his gun to her head. Suddenly, the Ghost Rider emerges from a portal beneath their feet. After making quick work of them, the girl runs away in fear of her.
The next day, at a base located in the desert, human traffickers, lead by the quite but ruthless Jones, prepare to ship a load of children to the United States. From the desert comes the Ghost Rider, who breaks in and frees the children. But Jones takes the truck that is still filled with children and drives away.
The Ghost Rider chases after him up the mountains until he steers the truck off a cliff and abandons it. Faced with the choice of saving the children or stopping Jones, the Ghost Rider chooses the children. After opening a portal that takes them to safety, the Ghost Rider hunts down Jones and corners him. It is then that Zarathos, the demon that inhabits the Ghost Rider, tells her that Jones is her father.
Changing back into Alejandra, she tells him she is his daughter, a fact that is unsurprising to him since he has gotten Mexican women pregnant before, and like her, they were sold off. As the Ghost Rider, Alejandra burns away all of her father's sin, leaving him alive but as an empty person.

In a Louisana swamp, the Ghost Rider encounters a mysterious old lady, who she deduces is the gatekeeper to the border of something across the swamp. After crossing the border, she changes back to Alejandra, but not of her own volition. Suddenly, she is attacked by an alligator, but is saved by a man on the other end of the swamp named Earle, who takes her to a village where killers are sent to change their ways and cannot leave until then.
The witch Alejandra encountered is the mother of a raped murder victim who swore revenge on the killers of the region. Earle is sure that Alejandra, being the only person she has ever allowed to enter the village, was sent to release them.
While Earle believes the villagers have changed for the better, that isn't the case. In a secret meeting, all of the villagers grab onto each other and begin to merge together, creating a single, monstrous person. They attack Alejandra and Earle and tell her that when the witch put them in the village she told them that the only way they could leave was if they killed someone, but found that it was impossible to kill each other. Alejandra on the other hand is not from the village, so it is possible to kill her. Escaping back into the swamp, Alejandra crosses the border and strikes them down as the Ghost Rider.
Earle is now able to leave the village and sees that the witch has decayed into a corpse, but the Ghost Rider kills him regardless of the fact that he believes he has changed.

Late at night in a Tokyo cemetery, Steel Wind and Adam, who somehow survived the space station explosion, revive Steel Wind's dead sister, Steel Vengeance, whose's still dead but with all of her robotics functioning. In return, the sisters must bring the Ghost Rider to him.
In a movie theater, the Ghost Rider asks the Seeker for help in putting the sin back into the Nicaraguans. The Seeker says that their sin can be found in Hell and Johnny Blaze could probably help her.
Meanwhile, Johnny is found and tied up by Hawkeye, a witness to the destruction in Nicaragua, who believes that Blaze as the Ghost Rider is responsible for it. After a brief fight, the Ghost Rider comes to his aid, but is suddenly sucked into a portal, along with Johnny.
The portal takes Johnny and Alejandra, now aware that he was a Ghost Rider, directly to Steel Wind and Steel Vengeance, who tells Johnny that the spell was only supposed to work on the Ghost Rider, meaning he may not be entirely rid of the curse.
The Ghost Rider challenges them but is surprised to learn the hard way that both sisters are a part of the bikes that they ride. On top of that, Steel Vengeance captures Johnny, and Steel Wind, feeling cheated out of their agreement because her sister is still dead, decides to leave with Blaze so that she can have her revenge against him.

Hawkeye explains to Alejandra that the amulet wrapped around the arrow stuck in her chest neutralizes magic. But now that she cannot do anything, it is up to him to save Johnny. Hawkeye rescues him just before the sisters smash him against a bullet train, but jumps off his sky-cycle, which flies towards the bullet train with Alejandra still tied to it. Alejandra is able pull out the arrow but the sky-cycle crashes into the train.
To make matters worse, Hawkeye is defeated and chained to the back of Steel Wind's motorcycle and dragged against the ground with the same thing happening to Johnny only on the back of Steel Vengeance's motorcycle.
However, the Ghost Rider follows behind them on top of the bullet train, now possessed as her vehicle. Johnny and Hawkeye climb onto it, giving the Ghost Rider a chance to blow the sisters up with a gun.
When the dust settles, the Ghost Rider prepares to deal the final blow to Steel Wind, who has been reduced to nothing but a head and torso, but Johnny convinces her to let her live and offers to help her. Though Hawkeye still wants her to pay for her crimes, Alejandra and Johnny teleport away, taking the magic paralyzing amulet with them.

The Ghost Rider bursts into a strip club and decapitates someone.
In the past, Alejandra and a boy escaped from the temple in Nicaragua at night. Alejandra tripped on a branch and tumbled with the boy down a hill, where they found themselves by a waterfall. The two then passionately kiss.
Back in the present, the head the Ghost Rider cut off belonged to the Seeker. The Ghost Rider orders him to give her more power so that she can break into Hell and take back the Nicaraguan's sin. The Seeker warns her that using the Ghost Rider for personal reasons will make Zarathos angry, but still informs her that Adam is the one who empowered her and he is still alive.
The Ghost Rider tracks him down and demands he give her more power. Proud that she is planning an assault on Hell, he happily obliges.
In the Nevada desert, just as Johnny is about to depart, Doctor Strange tells him that Alejandra is too weak to be allowed to possess the Spirit of Vengeance. Johnny knows that he must become the Ghost Rider again. Strange creates a portal for himself, Johnny, and several other heroes to travel through that will lead them to the Ghost Rider, but Johnny decides to go alone and shoots Strange in the foot so that he doesn't follow.
Outside the gates of Hell, a vastly powerful Ghost Rider defeats the demons guarding it and breaks her way in. Johnny enters Hell later on and finds it in ruins. Suddenly, he is surrounded by demons who threaten to kill him, but Johnny shows them his contract with Mephisto that proves he works for him. The demons let him go, but one of them mentions that his contract is a powerful weapon.
He proceeds onwards until he sees the Ghost Rider in the middle of a fight with Mephisto. Remembering what the demon said about his contract, Johnny takes it out again. Only this time, it turns into a bullet.
Despite her power upgrade, the Ghost Rider is still no match for Mephisto, who decides to kill her for attacking Hell. But before he does, he tells her that the reason he likes to involve himself with the Ghost Riders is because he finds it fun messing with a servant of God. Then, as he is about to strike her down, she reaches into his chest and rips out his beating heart. Now that she is in a position of power, she demands he give her the sins of the Nicaraguan people. He reminds her that reality will rip apart without a Hell, but she calls him on this, so he hands her a chest containing the Nicargua sins. She decides to kill him anyway, but she is stopped by Johnny, who fires the bullet into her skull, fullfilling his contract with Mephisto.
However, Alejandra changes back and falls off a ledge. She is able to save herself by grabbing onto a piece of the ledge, but she drops the chest in the process. Johnny comes to her aid, but she lets herself fall into the pit, believing that the people she was unable to help deserve vengeance. As she falls, a light shines around her and Johnny, who goes in after her. Shortly after, he emerges as the Ghost Rider with Alejandra in his arms.
In Nicaragua, the Ghost Rider tells Alejandra, whose skin is mostly burned and has a flaming skull like the Ghost Rider, that the huge amount of power that went from Alejandra to Johnny saved her when she fell into the pit of lava and some of that power is still in her. Alejandra calls herself a monster, so he shows her the Nicaraguan people, who now have their sin back, hoping this will make her realize she is not as bad as she believes. However, she is still mad at him for everything he put her through and rides away, declaring that his actions deserve vengeance. They both ride off in seperate directions.
